Attached template can be used to import questions in a learning environment with an English language pack.
Template was made for Multiple Choice questions. If you have questions with a singular answer, change MC to MA.
Step 1 Fill out the question information in the Word document table:

This table matches the information as asked upon adding a question in the Question Bank. (see below)


Step 2 What information goes where in the table?
- “Course name”: The name of the course.
- “Category”: The category of the course.
- Question text: The question.
- Answers A, B, C, D: The answers that can be selected.
After that there are a few other options that need to be configured:
MC/MA:
MC or MA has to be removed here, depending on the question type.
- MC: Multiple Choice (When only one answer is possible).
- MA: Multiple Answer (When multiple answers are possible).
Shuffle choices?

Yes: Answers will appear in a different order each time.
- No: Answers will appear in the order in which they were uploaded.
Number the choices?

0: No numeration.
- A: uppercase letters A,B,C,D
- a: lowercase letters a,b,c,d
- 1: numerical value 1,2,3,4,xxx
- i: lowercase roman numerical i,ii,iii etc
- I: uppercase roman numerical I,II,III etc
ID number: not a mandatory field in Moodle LMS, but has to be unique. Can be used to simplify the recollection of questions after creation.
Grade:
With one correct answer:
- Set the correct answer to the value of 100.
- Set incorrect answer to the values of 0.
With multiple correct answers:
- The sum of the values given to the correct answers needs to be 100.
(for 2 correct answers, both are given the value of 50).
- The sum of the values given to the incorrect answers needs to be -100.
(for two incorrect answers, both are given the value of -50).
Step 3: Importing multiple questions at a time
Multiple questions can be imported at the same time by copying and pasting the form, so long as you make sure each form (question) is on a new page in the Word document.
Step 4: Importing Word document in Moodle
In the Question Bank, go to Import:

Upon which this screen will show:

File format:
Select the option "Missing word format"
General:
Import category: Select the category in which you want the files to be imported.
Uncheck the box "get category from file"
Match grades: Set to "Error if grade not listed"
Stop on error: Set to "Yes"
Click "Choose a file..." and click the option "Upload a file -> Choose file" to find the Word document on your PC, then click "Upload this file".

Then click "Import" and the questions will be imported.
